Mid-Amateur Championship

All entrants, including exempt players, shall apply on LGA forms obtainable from LGA member clubs, LGA's website at www.LGAgolf.org, and at LGA office.

Entry fee of $200.  Entry fee must accompany application to be considered complete. There are no additional fees.  Entry fee will be refunded regardless of reason if LGA office is notified, in writing, before the close of entries. After the close of entries, fee will be refunded only in the event of injury or illness provided notification (accompanied by doctor's statement) reaches LGA office by August 6, 2009. $40 service charge will be deducted from all refunds.
