| Conditions/Schedules: | The LGA Tournament Committee reserves the right to alter any of the conditions and schedules herein. The decision of the Tournament Committee in any matter shall be final. |
| Dress Code, Cell Phones: | In all Sectional Qualifiers and Championship rounds play, competitors must be neat in appearance and wear appropriate attire. No gym shorts, cutoffs, blue jeans, tee shirts, etc. Shorts must be Bermuda length. Use of cell phones is prohibited on course(s) during competition. Important Note: Oakbourne Country Club and all qualifying sites are spikeless facilities and all competitors will be required to wear spikeless shoes. |
| Eligibility: | Entries are open to any Louisiana resident (as of at least 90 days prior to August 7, 2009) who is an amateur golfer (as defined by the USGA) holding membership in a LGA Member Club in good standing (2009 club and individual dues must be paid; club must be a part of the LGA Handicap Network or have USGA Handicap System Compliance Verification form on file in LGA office). Entrants must be 25 years of age by midnight, August 7, 2009, and possess a certified USGA Handicap Index of 8.0 or less. Non-amateurs are not eligible. Entries are subject to rejection at any time (including during Qualifying and/or Championship rounds play) by the LGA. Reasons for rejection may include unbecoming conduct. |
| Entry Deadline/Applications | All entrants must file an entry by 5 P.M., Tuesday, June 30, 2009. Entries must be filed online, via mail, or delivery to LGA office. Online entry applications can be completed at www.LGAgolf.org. Late or incomplete applications are not acceptable (Entries by FAX, telephone, or telegraph are not acceptable). Time limit for entries means time of receipt at LGA office, not time entries are sent to LGA. Entries should be submitted early to allow ample time for delay or error in transmission. The risk of delay or error in transmission lies solely with the entrant and the LGA will have no liability with respect to any such delay or error and the consequences therefrom, including rejection of the entry. All entrants must file an entry prior to deadline. Entry deadline is June 30, 2009, at 5 P.M. |
| Entry Fee, Withdrawal/Refunds: | Entrants shall apply on LGA forms obtainable from LGA member clubs, LGA's website at www.LGAgolf.org, and at LGA office. Entry fee of $200, plus cart fee for qualifying round only. Important Note: Entrants not successful in qualifying for Championship rounds play will receive a refund of $100 at the conclusion of the Mid-Amateur Championship. Entry fee must accompany application to be considered complete. If you qualify for Championship rounds play there are no additional fees. Starting times for qualifying rounds will be available by contacting the Golf Shop at your qualifying site after July 2, 2009. Entry fee will be refunded regardless of reason if LGA office is notified, in writing, before the close of entries. After the close of entries, fee will be refunded to those qualifying for Championship rounds play only in the event of injury or illness provided notification (accompanied by a doctor's statement) reaches LGA office by August 6, 2009. A $40 service charge will be deducted from all refunds. |
| Format of Play: | SCHEDULE: Competition will be in two stages, at stroke play. 1. Sectional Qualifying: 18 holes, for all except exempt entrants. Qualifying sites/dates as listed. 2. Championship Rounds: 54 holes. A field of 144 players will be accepted. Friday, August 7/First Round, 18 holes; Saturday, August 8/Second Round, 18 holes (60 lowest scorers and ties will continue play August 9; remainder eliminated after 36 holes). Sunday, August 9/Third Round, 18 holes. Lowest scorer for 54 holes will be 2009 LGA Mid-Amateur Champion. |
| Practice Privileges: | Practice round availability for qualifying events is at the discretion of the club hosting the qualifying event. Please contact the club where you will be qualifying for more information. Practice round privileges are extended to those qualifying for Championship rounds play at Oakbourne Country Club on August 6. Cart fee paid by LGA. |
| Prizes/Playoff: | Gift certificates will be awarded to the top 10 finishers in the Championship proper. The Mid-Amateur Champion will also receive a Crystal Trophy signifying the 2009 Mid-Amateur Championship. In case of a tie for the Championship, a sudden death playoff will take place immediately upon conclusion of play on Sunday. The top four finishers will receive invitations to participate in the 2009 Louisiana Cup Matches. |
| Rules of Golf: | Play will be governed by the 2008-09 USGA Rules of Golf and the current LGA Rules of Play Card. All questions will be settled by the LGA Rules Committee, whose decision will be final. |
| Schedule of Events: | On July 6, 7, 13, and 14, qualifying events will be held at eight sites across the state for all non-exempt entrants. You may obtain your starting time for your qualifying event by calling the Golf Shop at your qualifying site after July 2, 2009. Please call early as the Golf Shop at your site could be closed the day prior to the Qualifier. If successful in qualifying for Championship rounds play, you will receive additional information regarding the Championship. If unsuccessful, you will receive a $100 refund once Championship rounds play is complete in mid-August. |
